Full Life Christian Counseling looks forward to working with you! If you are a New Client, our Intake paperwork will be sent to you via your Client Portal. If you are coming as a couple, then each partner needs to have his/her own portal and complete his/her own forms separately.
The information collected from the Intake Papework and DSM-5 Assessment will help your counselor get a better glimpse of your history and goals for counseling.
If you are unable to complete your forms before your first appointment, then you will need to arrive at least 30 to 40 minutes early. Extra forms are available in the waiting area. The time allotted for the appointment cannot be extended due to incomplete forms.
